How to Apply
Download the application (PDF) by clicking on download application.
Print the application and fill it out. Please
print all information as completely as possible, including what your check cashing needs will be. Please supply the necessary
signature(s).
Turn in this application at the Service Desk of your closest Fairplay store. You will be requested to show your driver's license along with
other identification.
For personal checks - Your checkbook with current check number.
For government checks - A government I.D.
For payroll checks - A current payroll stub.
You will receive a notification from Fairplay that you may come in with your letter and additional identification to pick up your
new check cashing card. All other checks will only be cashed at the Service Desk.
